Rebate forms - this is the form you use to claim reimbursement from Cardiff Area for theatre claims, leisure centre claims,and when we advertise specific events where you can make a claim etc.
Please note with effect from the 1st May 2011 the Cardiff International Arena (CIA) will be limited to one ticket per member per month.

All claims need to be made within 1 month from the date of your event/visit.
Request Financial Support - use this form if you enter a specific CSSC sporting event and you want Cardiff Area to consider supporting your entry to the event.
Please note claims should be made after you have participated in the event.
